5/8/2023 EnchantaVaughn Approved (AM) Upon arrival, client was observed leaving off the daycare van with polite and appropriate manners. Client was respectful of his peers who were in front of him, allowing them to go ahead of him and enter the building safely. Client waved to QMHP and debriefed about the previous day. Client expressed positive thoughts about himself and how he was feeling. Client stated he wanted to take a small break before going to class. Client was encouraged to go to class and complete his work and then ask his teacher for permission to sit with his counselor. Client said okay and walked to class. Client was observed entering his class without hyperactive behaviors. Client went to his seat and prepared for morning announcements. Client seemed to be in a good mood, as evidenced by his positive interaction with peers and authority. Client expressed good comments about his classwork. Client was observed on task and engaging with his peers. Client offered to help others. Client exhibited good leadership skills and no redirection was needed at this time. Client displayed age appropriate behaviors and decrease of symptoms. Teacher reported client has had a good morning so far and supports his cooperation. Client was given praise and recognition for demonstrating responsible behavior. (PM) Client was cooperative going into the lunch room and demonstrating excellent social skills and exhibiting positive behaviors while waiting patiently in line. Client was respectful to staff, saying please and thank you. Client was witnessed treating others fairly. Client transitioned to psycho therapy smoothly and safely. Client displayed an acceptable level of participation and complying to instructions. Client was observed playing well with his peers and engaging in positive conversation. Client displayed good understanding during group intervention. Client was in a good mood. Client was observed practicing positive behaviors. Client reduced disruption by utilizing proper voice volume and staying on task. Client returned to normal activity without incident. (EOD) Client prepared for the end of the day by packing up and putting away his belongings. The client decreased impulsive behaviors. Client transitioned smoothly into academic setting. Client was observed listening to his teacher and following directions. Client was given a 30 minute break of relaxation. Client has shown improvement with meeting his objectives and benefits from TDT services due to reducing anger outbursts and controlling impulses. Client dismissed safely from school, no incidents were reported. Client was praised and rewarded for having a good day and achieving all objectives. (AM) QMHP inquired about weekend activities, asking what was exciting about your weekend? QMHP inquired about evening and morning and how client was feeling. QMHP offered help. QMHP encouraged client to have a good day. QMHP welcomed client to school. QMHP inquired about evening and morning. QMHP asked how client was feeling. QMHP asked if client needed help with anything. QMHP encouraged client to have a good day. QMHP monitored individualized behaviors for acquisition of objectives. QMHP encouraged client to participate in class. QMHP encouraged client to be the example for others. QMHP encouraged client not to follow others he see that are doing wrong. QMHP reminded client of goals and objectives. QMHP praised client for his efforts and encouraged him to remain on task. (PM) QMHP monitored for acquisition of objectives. QMHP redirected client negative behaviors. QMHP encouraged client to stay in class and comply to rules and directives. QMHP encouraged client to be the example. QMHP encouraged client not to follow others that he see are doing wrong. QMHP intervened to assist in explaining behaviors to client. QMHP processed client feelings. QMHP engaged in active listening. QMHP assisted client with recognizing his own personal strengths and how to apply them to reach his goals and objectives. (EOD) QMHP reflected with client on his day, reviewing both good and bad experiences. QMHP processed with client, "what would you do different tomorrowâ. QMHP encouraged client to have a good evening. QMHP encouraged client that tomorrow is a new day. QMHP praised client for utilizing coping strategies and being able to stay on track without redirection. QMHP processed client positive responses and challenged client to maintain appropriate behaviors throughout the entire week. (AM) Client had one on one with QMHP. Client reflected on weekend activities. Client said he woke up feeling good and ready to have a good day. Client expressed positive thoughts about himself. Client stated he spent time with his family yesterday and got plenty of rest. Client expressed he was ready for todayâs activities. Client shook his head and prepared for teacher instructions. Client was seen in compliance with all rules and regulations. Client stated he didn't need help with anything at the moment but would tell an adult if he needs help. Client communicated by nodding and exhibiting good eye contact. (PM) The client was provided with positive feedback as he displayed a realistic understanding of his recurrent pattern of mental illness symptoms. Client was able to stay in class without any incidents. Client said that he would try his best and looked forward to being a leader to his peers. Client has a clear understanding of goals and objectives and agrees to work on making better decisions. Client avoided distractions in social setting, while utilizing anger management skills. Client seem to have a big influence on his peers. Client displays leadership skills by helping others and being a good citizen in school. Client acknowledged interventions and verbalized clear understanding. (EOD) Client engaged in reflective thinking. Client communicated the do's and don'ts. Client was in a pleasant mood after feedback was given. Client was seen having a good evening. Client was able to identify the role he plays in each situation and continues to comply with authority rules and regulations. Client was reminded to apply himself and utilize his strengths to focus on meeting goals and objectives.
